We may collect non-personal information about Users whenever they interact with our Site. Non-personal identification information may include the browser name, the type of computer and technical information about how Users connect to our Site, such as the operating system and the Internet service providers used and other similar information. In general, we use non-personal Information to help us improve our Site and services and customize the User experience. We use the information that we collect to conduct our business and to improve your experience with our services. In order to do so, we also may combine any or all of the information that we collect or obtain.  Examples of how we use information are listed below. Any of the activities below will be performed consistent with our Notice of Privacy Practices (which is only applicable to information covered by HIPAA) and  Patient Agreement/Consent (if there is one between you and us). Note that the electronic communications described below may contain protected health information. You acknowledge and accept that such communications may be sent unencrypted and there is some risk of disclosure or interception of the contents of these communications.

 

  • To respond to you, such as when you send us an inquiry via e-mail or text message; fill out a form to be contacted; make a request or an inquiry; wish to set up an appointment with us; request materials in the mail; or share a comment or concern. 
  • To send you periodic emails or text messages for other reasons, such as to send updates about appointments and care. 
  • To provide you with services, such as to share or offer services that are relevant to your needs; deliver content that you may find interesting or helpful; and process, maintain, and service your account.
  • To personalize your experience with us, such as to remember your interests and preferences; customize the services and content we share with you; facilitate your interactions with Company representatives; track and categorize your activity and interests on our Site, including by identifying the different devices you use; and to enrich your experience with our services and Site.

Sharing Personal Information

We will only share your Personal Information with third parties as outlined in this Privacy Policy and as otherwise permitted by law or as permitted with your consent. 

 

We share Personal Information with vendors who are performing services for the Company, such as an email provider who may be provided access to User’s email address for purposes of sending emails from us. Similarly, we may hire third-party companies to host or operate some of our online services and related computer/software applications.Those vendors use your Personal Information only at our direction and in accordance with our policies.

 

In the event we undergo a business transaction such as a merger, acquisition by another company, or sale of all or a portion of our assets, your Personal Information may be among the assets transferred. You acknowledge and consent that such transfers may occur and are permitted by this Privacy Policy, and that any acquirer of our assets may continue to process your Personal Information as set forth in this Privacy Policy. If our information practices change at any time in the future, we will post the policy changes to the Site so that you may opt out of the new information practices. We suggest that you check the Site periodically if you are concerned about how your information is used.

 

YOUR RIGHTS AND CHOICES

We offer you choices about how Personal Information is used and shared. Your choices include:

Social Media. We encourage you to review your privacy options and settings with the social media platforms/networks you use to understand what choices you have about sharing information from those platforms and networks with us.

 

Mobile Location. You may choose not to provide your location by customizing your mobile device settings to prevent sharing.

 

Text Messages. You may opt out of receiving SMS (text) messages from us anytime by replying “STOP” to text messages received from us. You can also contact us at privacy@accompany.health and ask to opt-out of text messages.

 

Email. You can opt out of marketing emails by using the “unsubscribe” link or mechanism noted in marketing emails you receive from us. You can also contact us at privacy@accompany.health and ask to opt-out of e-mails. 

 

Please note that even if you opt out we may continue to send you administrative messages including, for example, periodic updates to our Privacy Policy.

 

NOTICES

Children. The Site and the Company services are not directed to anyone under the age of 13. The Site does not knowingly collect or solicit information from anyone under the age of 13, or allow anyone under the age of 13 to sign up for our services. In the event that we learn that we have gathered Personal Information from anyone under the age of 13 without the consent of a parent or guardian, we will delete that information as soon as possible. California Privacy Rights. This section applies to Users whose Personal Information we collect who are California residents. Please note that information covered by HIPAA is generally excluded from the scope of the California Consumer Privacy Act (“CCPA”). Please note that you have the right to request that we disclose the Personal Information that we have collected about you in the last twelve months, subject to certain exceptions. You also have the right to request deletion of Personal Information that we have collected about you, subject to certain exceptions. You also have the right to not receive discriminatory treatment for exercising your rights under the CCPA. Please note that unless you request us to or consent to us doing so, we do not share any Personal Information with third parties for their own direct marketing purposes. HOW WE PROTECT INFORMATION

We use industry best practice data collection, storage and processing practices and security measures to protect against unauthorized access, alteration, disclosure or destruction of your Personal Information and data stored on our Site. We further protect your information from potential security breaches by implementing certain technological security measures including encryption, firewalls and secure socket layer technology. We also take measures designed to ensure that Personal Information is only accessible to a limited number of personnel who need access to the information to perform their duties, and those personnel are required to keep the information confidential.

 

In general, information transmitted through the Internet may not be perfectly secure. As a result, we are unable to guarantee the security of Personal Information and we are not responsible for the circumvention of any privacy or security measures on our Site unless required by law. We are not liable for the illegal acts of third parties such as criminal hackers. As part of our services, we may provide links to or compatibility with other websites or applications. However, we are not responsible for the privacy practices employed by those websites or the information or content they contain. This Privacy Policy does not apply to your use of a third party website accessed by selecting a link on our Site or via our services. These third party sites and services may have their own privacy policies and customer service policies. The Company has the discretion to update this privacy policy at any time. When we do, we will revise the updated date at the bottom of this page.
